Fee schedule
A fee schedule is a list of the amounts a dental plan allows or pays for each service. It may differ from what a dentist usually charges.
A fee schedule is a list of services and the amounts tied to them. For a dental plan, the schedule of benefits lists dental services and the maximum benefit amount an insurer will pay for each. The details vary by plan.
The ADA explains that a benefit paid is often a percentage of the insurance company's customary fee schedule. Patients often do not know their out-of-pocket costs, since insurers generally do not release these customary fee schedule maximums to the public. A contracted dentist agrees to the plan's fees, as with an in-network dentist.
The amount on a plan's schedule can differ from a dentist's usual fee. Compare with UCR. Read your EOB to see what applied. The insurance and financing page has this office's information.
